Rockford Public Schools will once again offer sports physicals to all INCOMING seventh- through 12th-graders. The district hosts physicals just once per year and will cover the upcoming 2012-13 school year for middle school and high school athletes.
Any seventh- through 12th-grade student (fall of 2012) may participate at any location listed here:
• Tuesday, June 5 at North Rockford Middle School, beginning at 2:30 p.m.
• Wednesday, June 6 at East Rockford Middle School, beginning at 11 a.m.
• Thursday, June 7 at Rockford High School, beginning at 11 a.m.
A “parent”-completed physical form is required to participate. Forms are available at any secondary school office or on the high school athletic webpage at www.rockfordschools.org, under forms. The fee of $20 is payable to Rockford Athletics. For additional information, call the athletic office at (616) 863-6040.
